Some records related to substance use disorder (SUD) treatment have extra privacy protections under federal law (42 C.F.R. Part 2). These records cannot be used or shared without your written permission unless the law allows it. Using these records in court or legal proceedings requires your consent or a court order. 

Redisclosure 

Information shared by us may be shared again by others and may not be protected by federal privacy laws. SUD records keep their special protections unless the law allows otherwise or you give permission. 

Fundraising 

We may contact you about practice updates or fundraising. You can opt out of fundraising messages at any time. Your choice will not affect your care. 

Your Rights 

You can ask to see or get a copy of your records, ask for corrections, request limits on sharing, ask for private communications, and get a paper copy of this notice at any time.
